{"created":"2023-07-25T09:06:52.718563+00:00","id":5158,"links":{},"metadata":{"_buckets":{"deposit":"0895a58a-be09-42a7-8f44-e0de1b1b0095"},"_deposit":{"created_by":3,"id":"5158","owners":[3],"pid":{"revision_id":0,"type":"depid","value":"5158"},"status":"published"},"_oai":{"id":"oai:toyama.repo.nii.ac.jp:00005158","sets":["640:641:642"]},"author_link":["158856","158855"],"item_9_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"1994-02-09","bibliographicIssueDateType":"Issued"},"bibliographic_titles":[{}]}]},"item_9_date_granted_64":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"1994-02-09"}]},"item_9_degree_grantor_62":{"attribute_name":"学位授与機関","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_name":"富山医科薬科大学"}],"subitem_degreegrantor_identifier":[{"subitem_degreegrantor_identifier_name":"13202","subitem_degreegrantor_identifier_scheme":"kakenhi"}]}]},"item_9_degree_name_61":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士(医学)"}]},"item_9_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"麻酔導入薬として，超短時間作用性のパルビタールやケタミンの他に，ベンゾジアゼピン系薬物のジアゼパム，フルニトラゼパム，ミダゾラムが臨床使用されている。\nパルビタールは端息患者での使用が禁忌であることや呼吸および循環系に対する抑制作用が強く，ケタミンは麻酔中に悪夢を起こしたり血圧の上昇を起こすなどの欠点があるために，ベンゾジアゼピンを使用する機会も多い。\nミダゾラムは短時間作用性の水溶性ベンゾジアゼピンであり，筋肉内投与の麻酔前投薬として，またジアゼパムのような血管痛がないことから静脈内投与の麻酔導入薬として，近年繁用されている。\nベンゾジアゼピンは種々の程度の呼吸抑制を起こすことが一般的に知られている。\nさらに，ミダゾラムに関する副作用報告では，無呼吸を含む呼吸抑制が最も高率である。\nすなわち，ミダゾラムは臨床的に使用されるべンゾジアゼピンの中では最も強い呼吸抑制作用を示すものの一つであると考えられる。\nまた，ミダゾラム投与に関連した死亡事故における男性の死亡症例は女性のそれの3倍となっていること，ミダゾラム0.15mg/kgの静脈内投与による意識水準の低下および無呼吸発現頻度に関して，男女間に差があるとの報告もあり，ベンゾジアゼピンの感受\n性に男女差があることが推測される。\n一方，フルマゼニルはベンゾジアゼピンの括抗薬として開発され，ベンゾジアゼピンの大量服用時の中枢抑制状態や麻酔覚醒時の睡眠状態を効果的に改善することが知られている。\nしかし，フルマゼニルがミダゾラム投与下の炭酸ガス負荷換気応答を改善するという報告がある一方，逆に低下させるという報告もあり，ミダゾラムの呼吸抑制に対するフルマゼニルの括抗作用については充分に解明されていない。\n本研究は，健常人ボランティアにおけるミダゾラムによる呼吸抑制作用の特徴とフルマゼニルによる括抗様式を，胸郭・腹壁運動，鼻腔気流，呼吸性上気道雑音（いびき），頤部表面筋電図，呼吸数，動脈血酸素飽和度を指標として検討した。\nさらに，これらに関して，男女間に差があるか否かについても検討を加えた。","subitem_description_type":"Abstract"}]},"item_9_description_40":{"attribute_name":"資源タイプ(DSpace)","attribute_value_mlt":[{"subitem_description":"Article","subitem_description_type":"Other"}]},"item_9_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"富山医科薬科大学・博士(医学)・乙第229号・増田明・1994/2/9","subitem_description_type":"Other"}]},"item_9_dissertation_number_65":{"attribute_name":"学位授与番号","attribute_value_mlt":[{"subitem_dissertationnumber":"乙第229号"}]},"item_9_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{"nameIdentifier":"158856","nameIdentifierScheme":"WEKO"}],"names":[{"name":"Masuda,
