@article{oai:toyama.repo.nii.ac.jp:00002585, author = {八塚, 美樹 and 鴻巣, 聡子 and 今西, 信子 and 佐藤, 美友紀 and 片田, 裕子 and 上田, 京佳 and 寺西, 秀豊 and 落合, 宏}, issue = {2}, journal = {富山医科薬科大学看護学会誌}, month = {Sep}, note = {application/pdf, 富山県内の中学生以下の若年者を対象にラテックスアレルギーに関する調査を質問票により行った.405名の有効回答者の平均年齢は,12.6±2.7才(範囲1-15才)で,男児191名,女児214名であった.その45.1%に何らかの即時型アレルギーの既往を認め,特にラテックス含有製品により誘発されたラテックス感作群は80名(19.8%)であった.ラテックス感作群と他一般若年群間で平均年齢は,前群が4.6才若く,かつ他の即時型アレルギーの併発率及び即時型アレルギーの既往率は有意に高かった.主な誘発ラテックス製品として絆創膏(70.0%),家庭用ゴム手袋(11.3%),ゴム風船(11.3%)があげられた.主症状は皮膚・口唇症状が最も高く(78.8%),鼻・眼・喘息様症状(22.0~16.0%)であったが,4名にはアナフィラキシーを想起させる意識低下・血圧低下を認めた.症状持続期間は大半が1~数日であり,その間の原因製品の使用回避(36.6%)や医療機関受診(25.0%)等適切な対処法が取られていた.但し,本アレルギーの診断検査を受けた者は皆無で,さらに本アレルギーの認知度は7.8%と極めて低いことから,今後総合的啓蒙活動が必要と思われた.ラテックス特異的IgE陽性血清を用いたウエスタンブロット法で,このIgEは,ラテックスHe vB1とHevB7アレルゲンへの結合性が示されたが,ラテックスをトリプシンあるいはパパイン処理(37℃, 1時間)によりその結合性は失われた.このことより,蛋白分解酵素処理は,低アレルゲン化ラテックスの開発に有用であることが示唆された., We carried out an epidemiological study on the latex-sensitized children through the analyses of answers on the questionnaire obtained from 405 children with the mean age of 12.6 years in Toyama Prefecture. As much as 191 responders (45.1%) snowed either symptom of the immediate type allergy. Especially, 80 responders (19.8%) were classified into latexsensitized group (LSG) because their symptoms were induced by latex-related products such as sticking plaster, and rubber gloves and balloons. LSG was 4.6 years younger and showed a significantly higher prevalence rate of allergic diseases than the remaining 325 children. Although most of them suffered from cheilo-dermato, rhino-ocular and /or asthma-like symptoms with the duration time of 1 to several days, 4 children answered anaphylaxislike symptoms (low blood pressure and unconsciousness). To control these symptoms, they avoided the use of causative latex-related products or consulted with doctors, but no children received diagnostic tests of latex allergy. Most children and their parents (92.1%) have not known the latex allergy, suggesting that educational activities should be provided to inform them the increasing prevalence rate in children, potential risks and manifestations including lethal anaphylaxis. Western blotting analysis using a latex IgE-positive serum showed that two bands corresponding to Hev B1 and Hev B7, well-known allergens in latex proteins, could be detected before the treatment of latex proteins with trypsin or papain (37℃, 1h), but not after the treatment.
