 note = {目的：終末期がん患者と家族とのあいだで，療養先の選択について意見が相違しているとき，がん看護専門看護師はどのように調整しているのかについて明らかにする．
方法：研究デザインは，質的記述的研究である．病院で勤務するがん看護専門看護師5 名を研究参加者として，半構造化面接法を用いて面接し，能智の分析手法を参考に，捉え方と調整について分析した．
結果および考察：がん看護専門看護師は，療養先の選択において意見が相違している終末期がん患者と家族が死と向き合うことに躊躇している捉え方に対し，死は生きることの自然な流れであることに気づくように調整していた。また，過去から現在の患者と家族を捉え，ありのままの姿を受け入れる調整，患者が在宅へ帰ることが必ずしも良いとは限らないと，患者と家族を一つの単位とした捉え方に対し，双方の合意点を見出す調整，長年共に生活する中で培ってきた家族の持つ潜在的な力を捉えていることに対し，その力を引き出し高めることでQOL 向上を目指した調整が重要であると考えられた．, Purpose of the study：To perceive and analyze the actual coordination efforts of the cancer terminal care nurses when the discrepancies occur between the patients and their families about where to receivethe terminal care.
Methodologies：Implementing the qualitative and descriptive interview method. Depicting 5 cancer terminal care nurses as the interviewees and implemented semi-structural questions to perceive and analyze their arbitrational skills by comparing with their already known terminal care skills.
Results and the evaluations：Certified nurse specialist in cancer nursing have arbitrated and encouraged the patients and their families who suffer the discrepancies of the opinions about where to receive the terminal care by letting them appreciate and accept the deaths as the natural course of the life. Also, they have perceived the patients and their families not as one unit but two different entities having shared the same history from the past to the present. So, the different entities don’t necessarily have to agree with the decision to receive the terminal care at home. The nurses have noticed several potential solutions for each and every family because they have different background histories.
