quality","subitem_description_type":"Abstract"},{"subitem_description":"国立社会保障・人口問題研究所は，2017（平成29）年8月1日に「社会保障費用統計平成27年度」を発表し，2015（平成27）年度の社会保障給付費（国民の自己負担分などは除く）が前年度比2.4％増の114兆8596億円で過去最高になったとした。最も伸びが大きかったのは医療分野で，前年度比3.8％増の37兆7千億円に達し，今後の持続的な社会保障制度の構築のために，負担と給付の見直しが急務としている。\n最初に退院計画を導入し制度化したのはアメリカ合衆国である。1982年に公的保険制度であるメディケア・メディケイドに，診断群別の包括医療費制度であるDRG/PPS が導入され，同じ診断であれば安い治療で早く退院させる傾向が強まり，治療が不十分なままの退院が社会問題化した。アメリカ病院協会（American Hospital Association）は1983年に「退院計画のガイドライン」（Introduction to Discharge Planning for Hospitals, American Hospital Publishing）を発行し，退院計画について定義をしている。\n我が国においても，医療費抑制と早期退院は1990年代から盛んに議論されるようになった。二木立は，医療費と入院抑制の関係性を否定している。しかしながら，政府は一貫して，医療費の抑制策として，平均在院日数の抑制を行っている。\n本研究は，増大している社会保障給付費の中の医療分野に着目し，特に医療費の抑制策として近年，国が力を入れている入院医療費の課題について，地域包括ケアシステムの構築と地域完結型医療，それを含んだ退院支援に関し，社会福祉の視点から考察を試み，特に急性期病院においての退院支援にソーシャルワークアセスメントの重要性に着目し，アセスメント試案を検討するものである。\n尚，本稿の中で，保健医療機関のソーシャルワーカーを，ソーシャルワーカー又は医療ソーシャルワーカーと表記し，診療報酬上で国家資格者として規定されているものを社会福祉士と表記している。","subitem_description_type":"Abstract"}]},"item_3_description_40":{"attribute_name":"資源タイプ(DSpace)","attribute_value_mlt":[{"subitem_description":"Article","subitem_description_type":"Other"}]},"item_3_description_6":{"attribute_name":"引用","attribute_value_mlt":[{"subitem_description":"富山大学人間発達科学部紀要,
