The growth process of new graduate nurses in nursing practice

目的:看護実践における新人看護師の成長のプロセスを明らかにする.
方法:新人看護師7 名を研究協力者とし,半構成的面接で得たデータを修正版グラウンデッド・セオリー・アプローチ(M−GTA)により分析した.
結果:25 の概念が生成され,12 のサブカテゴリー,4 つのカテゴリーが抽出された.新人看護師は病棟に配属されて《リアリティショックを経験する局面》に至るが,同時に仕事,患者,同僚らと〈職場でのつながりを持つ〉ことを起点として,患者との関わりを深め,病棟で仲間意識をもって《社会化に直面する局面》を進んでいた.さらに看護体験から《振り返りと再構成の局面》および《向かうべき方向性の追求》へとつながって成長するプロセスを,コアカテゴリーとして【キャリア初期における役割移行】と捉え,「新人看護師のキャリア初期における役割移行モデル」と命名した.
結論:本研究により明らかになった「新人看護師のキャリア初期における役割移行モデル」は新人看護師の成長の状況を把握し,より効果的な継続教育や支援をするための視点として活用できる.
Purpose: To clarify the growth process of new graduate nurses in nursing practice.
Methods: Semi-structured interviews were conducted with 7 new graduate nurses who cooperated in the
study, and the obtained data were analyzed with M-GTA.
Results: Twenty-five concepts were formed, and 12 subcategories and 4 categories were identified. New
graduate nurses reached a “Phase of experiencing reality shock” when they were assigned to a ward, but
at the same time they progressed in the “Phase of directly confronting socialization” by deepening their
involvement with patients and having a sense of camaraderie on the ward, the starting point for which
was Relations in the workplace with the work itself, patients, and colleagues. Nursing experience also led
to a “Phase of reviewing and reconstructing” and “Pursuing the direction in which one should go.” Role
transition in the initial career stage was understood to be a core category, and named Role Transition
Model in the Initial Career Stage of New Nurses.
Conclusion: The model demonstrated in this study apprehends the growth status of new nurses and can be
used as a vantage from which to provide more effective continuing education and support.
